Dans cette revue systématique secondaire d'études d'intervention à cas unique et en groupe contrôlé menées auprès d'adolescents autistes, nous avons examiné les caractéristiques de 48 études portant sur 273 participants qui mesuraient au moins un résultat de "comportement problématique" (numéro d'enregistrement Prospero : 231764). Nous avons recherché des études pertinentes dans 11 bases de données et la date finale de la recherche était novembre 2022. Nos principaux objectifs étaient de déterminer comment les comportements problématiques étaient définis et sélectionnés pour être réduits, comment les fonctions étaient déterminées et les interventions utilisées pour y remédier. Les études ont été codées et les codes ont été tabulés et convertis en pourcentages pour répondre à chaque question de recherche. Trente-huit pour cent des études définissaient un comportement problématique et 88 % des études mettaient en œuvre des stratégies comportementales pour réduire les comportements problématiques. Les comportements à faible potentiel de préjudice constituaient la majorité des 67 variables de résultat (61 %), tandis que les comportements à fort potentiel de préjudice constituaient une minorité (39 %). La cible d’intervention la plus courante était le comportement stéréotypé. Moins de la moitié des études ont signalé des procédures de sélection des comportements, des procédures signalées pour déterminer la fonction comportementale ou des fonctions attribuées aux comportements. Nous n'avons pas été en mesure de rendre compte de certaines caractéristiques démographiques des participants (par exemple, race/origine ethnique) car elles étaient rarement rapportées dans les études primaires. Nous concluons que les comportements problématiques sont mal conceptualisés dans cette recherche.

In this secondary systematic review of single-case and controlled group design intervention studies conducted with transition-age autistic youth, we examined features of 48 studies with 273 participants that measured at least one “problem behavior” outcome (Prospero registration number: 231764). We searched 11 databases for relevant studies, and the final search date was November 2022. Our primary aims were to determine how problem behaviors were defined and selected for reduction, how functions were determined, and the interventions used to address them. Studies were coded and codes were tabulated and converted to percentages to answer each research question. Thirty-eight percent of studies defined problem behavior, and 88% of studies implemented behavioral strategies to reduce problem behaviors. Behaviors with low potential for harm constituted the majority of the 67 outcome variables (61%), while behaviors with high potential for harm were a minority (39%). The most common intervention target was stereotypic behavior. Fewer than half of studies: reported procedures for selecting behaviors, reported procedures to determine behavior function, or ascribed functions to behaviors. We were unable to report on some demographic features of participants (e.g. race/ethnicity) because they were rarely reported in primary studies. We conclude that problem behavior is poorly conceptualized in this research.
