L'hypothèse du monotropisme postule que l'hyper-concentration sur les centres d'intérêt est au cœur de la cognition autistique ; de plus, l'hyper-concentration est courante dans le développement avec dysrégulation de l'attention et hyperactivité (TDAH). Cependant, le TDAH est en partie défini par une susceptibilité à la distraction, et la recherche a mis en évidence une capture de l'attention dans l'autisme. Nous avons examiné l'hyper-concentration dans l'autisme et le TDAH, sa relation avec l'inattention, et si elle est liée à la qualité de vie, l'hypervigilance, les pensées répétitives négatives, ainsi qu'aux symptômes d'anxiété et de dépression. Un total de 492 adultes (122 ayant seulement le TDAH, 130 seulement autistes, 141 autistes + TDAH et 99 en groupe de comparaison) ont complété des questionnaires indexant l'hyper-concentration dans plusieurs domaines, l'inattention et la valence de leurs expériences d'hyper-concentration. L'hyper-concentration et l'inattention étaient élevées dans tous les groupes neurodivergents et étaient positivement corrélées, suggérant que la régulation atypique de l'attention, se manifestant parfois par l'hyper-concentration et parfois par l'inattention, est un phénomène trans-diagnostique. L'hyper-concentration prédisait une qualité de vie globale inférieure, plus d'hypervigilance, plus de pensées répétitives négatives et davantage de symptômes d'anxiété et de dépression ; cependant, des expériences positives d'hyper-concentration médiatisaient une relation indirecte entre l'hyper-concentration et une meilleure qualité de vie, atténuant partiellement l'association négative globale. En effet, les participants neurodivergents ont non seulement rapporté davantage d'expériences négatives d'hyper-concentration par rapport aux témoins, mais également plus d'expériences positives. Des études futures pourraient explorer les facteurs contextuels influençant si l'hyper-concentration a des impacts positifs ou négatifs sur la vie quotidienne.
